Across our East Health Trust network of clinics, new healthcare roles have been introduced to make it easier for you to get the right care at the right time.

These include Integrated Care Co-ordinator Nurses, Extended Care Paramedics, Health Improvement Practitioners, Health Coaches, and Clinical Pharmacists.

This team-based approach means your care is shared and coordinated across highly trained professionals who work together – and with you.

Whether you need support with mental well-being, managing medications, staying active, or navigating long-term health conditions, there’s someone on the team who can help.

These changes mean shorter wait times, more specialised support, and a better understanding of your needs.

For example, you might meet with a Clinical Pharmacist to review medications, or a Health Coach to set achievable health goals – all within your local clinic.
